Complete a short screening or speak with a Care Navigator.

Review Medicaid

Review your plan, waiver, and any personal-care hours already approved.

Assess Care Needs

The assessment and care plan identify needs, subject to authorization.

Prepare Together

Eligible caregivers complete employment requirements, training, and visit verification setup.

Begin Authorized Care

Begin approved services with ongoing support for the care plan.
