A daughter and her mother discussing support at home

For clinical and community partners

Help a family find their next step.

Connect families with personal care, established home-health services, and guidance on paid family caregiving through Virginia Medicaid.

When to connect

A practical handoff for families who need care at home.

A conversation may be useful when a child or adult needs hands-on help with bathing, dressing, toileting, eating, transfers, or mobility—and the family needs guidance on arranging that care.

For paid parent caregiving, the child must need extraordinary personal care beyond what is normally expected at the same age. A diagnosis, supervision need, or referral alone does not establish eligibility.

Start with your contact details

Use the form below for your professional contact information, organization, and service area. It does not ask for a patient’s name, health details, identifiers, or records.

Arrange the handoff first

Call to confirm the appropriate secure referral method and any required consent before sharing protected information. Do not send clinical details through this form or ordinary email.

Clarify the next steps

First Choice can review service fit and the family’s coverage questions, explain the assessment and authorization process, and discuss caregiver onboarding when applicable.

Agree on follow-up

Tell our team your preferred contact method. When you connect, ask for the Care Navigator handling the referral and agree on a follow-up time. Call 571-325-2340 to check on a request.

A family can start in their own words.

The preliminary eligibility check asks for broad care needs and contact details. It does not guarantee Medicaid approval or service authorization.
