Who to contact
This website is presented under the First Choice Home Care name. The precise legal entity responsible for website data and its designated privacy officer must be confirmed before launch. Please include a contact method and the nature of your request, without medical records, identity documents, or other sensitive attachments.
Information you choose to provide
- Care inquiries: your name, ZIP code, service of interest, contact details, preferred contact method, language, and contact permission help the team respond and identify a next step.
- Eligibility screening: location, the care recipient’s age range and relationship to the caregiver, selected daily-care needs, existing care, Medicaid and plan or waiver status, existing authorized hours, and whether the request is to start or transfer services support a preliminary review. Name, contact preferences, language, and a preferred contact time help arrange follow-up. These answers can be sensitive even without a diagnosis.
- Professional referral contact: professional contact details, organization, role, ZIP code, language, and contact preferences help arrange a referral discussion. The public form does not request patient names, clinical details, records, or uploads.
- Caregiver applications: contact and address details, work eligibility, position and availability, qualifications, experience, work history, professional references, acknowledgments, and the optional documents offered by the form support hiring review. Obtain permission before sharing another person’s reference information.
- Submission records: a reference number, submission date, consent version, source page, campaign parameters when present, and internal review status support receipt, follow-up, and operational reporting.
The initial care and eligibility forms do not ask for a Social Security number, Medicaid member number, exact date of birth, or medical records. Do not put health information in campaign links or unrequested fields. A preliminary screening does not establish Medicaid eligibility or service authorization.
Systems that process information
The current implementation uses Cloudflare Pages to serve the website, a Cloudflare Worker to process submissions, Cloudflare D1 to store intake and application records, and Cloudflare R2 for application documents. Clerk is used for team sign-in. Requests to hosting and authentication providers may include network and browser information such as an IP address, request time, device or browser details, and requested URL.
Staff review screens require authentication and server authorization. The final account owner, authorized users, service-provider terms, data locations, and any required business associate agreements have not yet been confirmed for production. This notice does not establish that the current preview is an approved channel for protected health information. Contact the team to arrange an approved process before sending clinical records.
Browser storage, cookies, and analytics
Unsubmitted application answers and selected files stay in the current page’s memory. Refreshing or closing the page can lose that work. The application removes its old saved-draft key when it opens and no longer saves application drafts in browser storage.
The website uses session storage for landing-page, source-page, and campaign attribution during the browser tab session. This attribution storage does not contain form answers or documents. Staff sign-in may use authentication cookies and related storage. Fonts and site images are hosted with the website.
No advertising pixel, general analytics SDK, session replay, or call-tracking script has been added to this website’s code. Hosting and account-level tools must also be checked before launch. A proposed measurement plan remains subject to privacy review; this statement must be updated before new tracking is activated.
Contact permission and text messages
Contact permission is for responding to the request you submit. Where the form offers text messages, choose that method only if you want texts about your request. Message frequency would depend on the conversation, and carrier message and data rates may apply. You may withdraw contact permission by calling or emailing the team.
Automated SMS delivery is not configured in the current implementation. Before an SMS program is activated, First Choice must approve its provider, sender identity, frequency description, and working STOP and HELP handling and update the form disclosure. The website’s receipt confirms storage; it does not mean a text or email has been delivered or a staff member has reviewed the request.
Security, retention, and deletion
The code includes submission validation, authenticated staff access, and file size and type restrictions. These controls alone are not a statement of HIPAA compliance or a guarantee of security. The production encryption configuration, upload scanning, access reviews, backups, logging, and incident response need verification.
No approved record-retention schedule or automatic deletion process is established in this website code. First Choice must confirm the periods and deletion process for inquiries, screening records, applications, uploaded documents, logs, and backups before launch. Clearing browser storage does not delete a submitted record from the server.
Privacy requests
You can contact the team to ask what information it holds, request a correction or deletion, change contact preferences, or raise a privacy concern. The applicable rights, identity-verification process, and any exceptions depend on the records and law that apply; this draft does not promise a particular response period or deletion outcome. Use the reference number if you have one. The team should arrange a suitable verification channel before requesting sensitive identity information.
