How to request language or communication assistance and raise a concern about access to First Choice Home Care.

Respect and equal access

First Choice’s proposed commitment is to treat people respectfully and provide access without unlawful discrimination. The final notice must identify the protections and procedures applicable to the organization, including language and disability access, and must be approved by the responsible compliance adviser.

Ask about language or communication assistance

Call 571-325-2340 or email info@firstchoicehome.care. Tell the team your preferred language or communication format and the kind of help you need. The eligibility and professional-contact forms also ask for preferred language.

The team must confirm how qualified interpretation, translated information, auxiliary aids, and alternative formats will be arranged, including any assistance required at no cost. Professional Spanish translations of the caregiver and eligibility pages are planned for review; translated versions are not yet available here.

Raise an access concern

Use the phone number or email above to request the responsible person for an access or discrimination concern. The named grievance coordinator, mailing address, and complaint procedure must be confirmed for the final notice. Please do not send health records by ordinary email.
