Two different starting points for a Medicaid screening.
Fairfax County directs CCC Plus screening requests for children under 18 to Coordinated Services Planning at 703-222-0880. Adults 18 and older are directed to Aging, Disability and Caregiver Resources at 703-324-7948.
A screening request is separate from asking a home care agency about services. Tell the county whether the request concerns a child or an adult so it reaches the appropriate team.
For developmental disability waiver questions.
The Fairfax-Falls Church Community Services Board handles developmental disability intake for Fairfax County and the cities of Fairfax and Falls Church. The dedicated intake page lists 703-324-4400, TTY 711, for appointments.
A DD waiver assessment is different from the CCC Plus screening route. If you already have a support coordinator, ask about the current waiver status and any personal-care authorization before arranging an agency change.
Home care options for your family.

Already caring for a family member?
Virginia Medicaid may allow an eligible family member to provide authorized personal care as a paid caregiver. For the parent or guardian of a minor, the LRI rules focus on extraordinary hands-on ADL needs beyond what is typically expected for a child of the same age. A diagnosis alone does not establish eligibility.
A qualifying paid LRI may receive up to 40 reimbursable hours per week for a child, subject to assessment, the care plan, authorization and applicable program rules. General supervision and ordinary parenting alone do not qualify. Ask how the LRI respite restriction affects any existing care arrangement.

Who should I contact for a child’s CCC Plus screening in Fairfax County?
The county directs requests for children under 18 to Coordinated Services Planning at 703-222-0880. DD waiver intake uses the separate Fairfax-Falls Church CSB route. If you are unsure which assessment is appropriate, explain the child’s care needs when you call.
Does a CSB intake appointment mean personal-care hours are approved?
No. Intake and eligibility assessment are part of the public-service process. Waiver enrollment, availability of a DD waiver slot and authorization of particular services are separate questions. Ask your coordinator what is currently approved before starting paid care.
Can I discuss home care while waiting for Medicaid answers?
Yes. You can ask First Choice about the type of care you need and current local availability while the coverage questions are being resolved. Medicaid payment and a paid family caregiver role still require the applicable eligibility, assessment and authorization.
