Loudoun County’s Medicaid Personal Care page describes help at home as an alternative to nursing-home admission for people who meet the requirements. Its Family Services contact is 703-777-0353.
Explain that you are asking about in-home personal care and whether a screening is needed. The county’s public-program assessment does not itself confirm First Choice availability or employment for a family caregiver.

Loudoun’s Same Day Access page gives a separate number, 703-777-0597, for screening for Developmental Disability Medicaid waiver eligibility. This is distinct from the general same-day behavioral-health clinic.
If the goal is a DD waiver assessment, say that when contacting the county. Do not assume a general walk-in assessment enrolls a child or adult in a waiver or creates approved personal-care hours.

Home support may involve help with bathing and mobility, clinical nursing, therapy goals or companionship. Describe the needs and the preferred schedule together so the team can discuss appropriate care, availability and any referrals or coverage checks required.

Already caring for a family member?
Virginia Medicaid may allow an eligible family member to provide authorized personal care as a paid caregiver. For the parent or guardian of a minor, the LRI rules focus on extraordinary hands-on ADL needs beyond what is typically expected for a child of the same age. A diagnosis alone does not establish eligibility.
A qualifying paid LRI may receive up to 40 reimbursable hours per week for a child, subject to assessment, the care plan, authorization and applicable program rules. General supervision and ordinary parenting alone do not qualify. Ask how the LRI respite restriction affects any existing care arrangement.

The county’s Same Day Access page directs DD Medicaid waiver eligibility screening inquiries to 703-777-0597. Use that specific route to ask about a waiver assessment; the general behavioral-health clinic is a different service.

Can an existing Loudoun waiver approval be used with a new agency?
The proposed agency, service and authorization need to be checked. Tell First Choice that a waiver or hours are already approved and involve the care or support coordinator before changing the current arrangement. A new start date should be coordinated rather than assumed.
