Alexandria Aging and Adult Services lists 703-746-5999 for long-term care assessment requests. Its guide describes assessments by a family services specialist and a public health nurse for residents seeking home-based Medicaid waiver care.
The city also explains that financial eligibility for Long Term Care Medicaid must be addressed. A care-needs assessment and the financial application are separate parts of the process.
A dedicated route for children and youth under 22.
The Children and Youth Developmental Disability Program serves qualifying city residents under 22. The dedicated program page lists 703-346-9893 for youth intake.
The city distinguishes a DD waiver screening through the Community Services Board from a CCC Plus screening through the local Health Department. Tell the team what coverage and services are already in place so it can direct the request appropriately.

Virginia Medicaid may allow an eligible family member to provide authorized personal care as a paid caregiver. For the parent or guardian of a minor, the LRI rules focus on extraordinary hands-on ADL needs beyond what is typically expected for a child of the same age. A diagnosis alone does not establish eligibility.
A qualifying paid LRI may receive up to 40 reimbursable hours per week for a child, subject to assessment, the care plan, authorization and applicable program rules. General supervision and ordinary parenting alone do not qualify. Ask how the LRI respite restriction affects any existing care arrangement.
